Group Training Payment Policies
Auto Payment Policy
Cancellation requires a 15 days written notice…email to firstname.lastname@example.org or mail request to Phase 5 Fitness, 202 S. Moon Ave. Brandon, FL 33511.
If you do not meet the required 15 day cancelation notice prior to your next billing date you will be charged on that date. In this scenario you will be allowed to attend group training classes for the remainder of your billing cycle. Upon completion of your last billing cycle, training shall then be considered terminated. If you decide to rejoining Phase 5 Fitness after the termination of training, payment will be at the current rate.
Per your auto-debit agreement, your card on file will be charged on your start date each month, unless otherwise agreed upon with Phase 5 Fitness. Cards should be kept up to date on your enrollment profile. In the event that your credit or debit card is declined you will have 3 business days to provide a new form of payment. If a new card is not provided within the 3 day period a 10% merchant service fee reimbursement will be charged to your account.
If refunds are agreed upon after an auto payment has run you will be credited less a 10% merchant services fee reimbursement. In the case of agreed upon refunds they will be issued at Phase 5 Fitness discretion…no later than 30 days from the agreed upon refund date.
Prepaid Group Training Packages:
At Phase 5 Fitness we offer discounted prepaid training packages…in an effort to provide our clients increased value. These prepaid packages auto renew in the same fashion as our month-to-month training fees. However, we require a 45 day written notice of cancelation to be emailed to email@example.com for these prepaid programs.
As a small business we budget based on projected revenue thus requiring advanced notification for these discounted programs. If the 45 day cancelation notification is not received within that time frame a cancelation fee equal to 20% of the prepaid agreement total will be applied and the clients training will end immediately.

It is the sole responsibility of the client to be aware of the renewal date of their prepaid program. If you do not know your renewal date please email eric@phase5fitness requesting that information.
In summary, there are no refunds for agreed upon training. However, all clients have the option to put their membership on hold one time for up to (1) month per calendar year, at no charge, for any reason. Upon expiration of Hold Date, your account will automatically become active and payments shall resume. Should you elect to cancel during the Hold Period, you will be subject to the 15 or 45 day Cancellation Policy per your auto debit agreement. Holds requires a written notice- emailed to firstname.lastname@example.org. You must request holds 15 days before next draft date for changes to be effective for that draft date.
LOST/STOLEN ITEM POLICY:
Phase 5 Fitness can not be held responsible for the loss or theft of personal items left in the gym or in the car parked on our premises.
In short, there are no refunds. However, if a refund is agreed upon, it will be subject to a 10% processing fee.
PERSONAL TRAINING and FME SESSIONS:
Sessions must be cancelled at least 12 hours prior to appointment otherwise the session will be fully charged without the ability to make it
